This document is an excerpt from the publication Measuring Performance: A Guidance Document for Promise Neighborhoods on Collecting Data and Reporting Results. It includes Appendix 6.6 and 6.7, a model data security plan and staff confidentiality statement.  Although written for Promise Neighborhood grantees, it provides an example for community groups to think through and document their procedures for safeguarding sensitive data.
